As of August 1, 2024, the Centers for Disease Control and Prevention (CDC) have introduced new regulations for importing dogs into the United States. These updated rules aim to prevent the reintroduction of canine rabies and ensure the health and safety of pets and the public. A crucial requirement under these new regulations is the CDC Dog Import Form, which must be completed before arrival.

What is the CDC Dog Import Form?

The CDC Dog Import Form is an online document that gathers important details about the dog, including its vaccination history and country of origin. Anyone bringing a dog into the U.S. must complete this form in advance. The form is available on the official CDC website and should be presented upon arrival.

Requirements Based on Country Risk Status

Dogs entering the U.S. are categorized based on their country of origin’s rabies risk level. The requirements differ accordingly.

Dogs from Rabies-Free or Low-Risk Countries:

  • Submit the CDC Dog Import Form online before travel.
  • The form receipt is valid for multiple entries over six months unless the dog travels to a high-risk country.
  • The dog must be at least six months old, microchipped, and appear healthy.

Dogs from High-Risk Countries:

  • Submit the CDC Dog Import Form before travel.
  • The form receipt is valid only for a single entry on the specified arrival date.
  • Provide proof of rabies vaccination, a valid rabies titer test, and a microchip.
  • If required, have a reservation at a CDC-registered quarantine facility.

How to Complete the CDC Dog Import Form

The form requires the following details:

  • A recent photo of the dog (taken within 15 days of travel if under one year old).
  • The dog’s microchip number.

After submission, a receipt will be sent via email. Travelers must present this receipt at check-in and upon arrival in the U.S. It can be printed or displayed on a mobile device.

Additional Considerations

  • Each dog requires a separate form.
  • Forms should be submitted well in advance of travel.
  • State and USDA regulations may apply in addition to CDC requirements.
